We do not recommend one insurance over any other. You should consult with your insurance agent to decide which insurance is best for you.

INSURED PATIENTS
Copays are expected the day of your visit. We bill your insurance, then bill you based on what your insurance company has said is your responsibility. If you have secondary insurance, we will only allow 45 days after we bill your secondary insurance before we will hold you responsible for payment.

UNINSURED/SELF-PAY PATIENTS
Patients without insurance ("self-pay") are requested to make an $87 payment prior to being seen. We offer a Self-Pay Discount of 30% for payment rendered on the date of service. Your deposit will be deducted from your total should you choose to make your payment in full upon checkout. Services not paid at checkout will be billed in full and payment will be expected within 30 days of your visit.

*NOTE: This practice serves all patients regardless of ability to pay. As an approved site of the National Health Service Corps, we also offer a sliding fee scale, where discounts for essential services are offered depending upon family size and income.

 

Application forms may be requested from one of our front office staff. They are also available for download on our Patient Forms page. Applications must be completed in full and submitted to our office with the appropriate documentation to be considered. If you qualify, your discount is good for 1 year, at which time you must reapply.
